Bank-in payment slip for the application Fee of RM50 (non-refundable) payable to Pusat Kembangan Pendidikan , CIMB Account number : 12180008810055. Please state your full name and I/C Passport No at the back of the bank slip. |

1 set of certified true copies of all academic certificates together with academic transcripts |

1 latest passport size photograph (non-refundable) |

A certified true copy of identity card or passport |

Original offer letter from your sponsorship/ education fund assistance, etc if applicable |
