Notice of Application for Reduction or Exemption in Charge Rate for Elective Dental Procedures

We are pleased to inform our patients that a reduction and/or exemptiom in the charge rate is available for selected elective dental procedures. This reduction is designed to help make dental care more affordable and accessible.
